That is the extent of the official description. Two things follow from it that are worth noting:
The portals are in the open world. This is not a menu-selected mode like the Tower of Trials. Rifts appear across the Continent of Orbis, which gives the open world a reason to be re-traversed after the story ends — currently one of its weaker points.
"Alternate realms" implies distinct spaces, not reskinned field encounters. How distinct is unknown.

Team composition will still decide outcomes. Every mode in this game rewards covering setup, break and burst, because that is a property of the combat system rather than of any individual mode. A trio that stalls at Tower floor 25 will stall in Rifts too. Check yours with the team builder.
Karma will still be your largest power source. Whatever Rifts drop, matching a Karma card's element to its hero will still be the difference between a card working fully and working partially.
